Most OB/GYNs and primary care providers are generalists — perimenopause and menopause represent a small slice of what they manage. At Evangeline Midlife, this is the only thing we do. Erin brings over 13 years of specialized women's health experience, advanced certification in menopause care, and the dedicated time to actually listen and build a personalized protocol around your full picture — not just your labs. Everything is tailored to help you move forward with clarity and confidence.

Evangeline Midlife is a cash-pay practice and does not bill insurance. This model allows Erin to spend significantly more time with each patient and provide the kind of individualized care that a volume-based insurance model rarely allows. She does allow you to use your insurance for your labs and commercial medications- with the understanding that coverage varies. For those not wanting to use insurance, Erin does offer convenient cash-pay pricing as well. Many patients find that the clarity of transparent pricing — combined with the quality of care — is well worth it.

The office is located at 5620 Tara Blvd., Suite 203, Bradenton, FL 34203. Erin sees patients in-person from Bradenton, Sarasota, Lakewood Ranch, Palmetto, Ellenton, Parrish, and surrounding communities. Telehealth appointments are available however, for patients throughout Florida.

Evangeline Midlife does not offer free consultations. The New Patient Package ($397) includes two dedicated appointments - an initial intake visit and a lab review/treatment planning visit - designed to give you a complete, unhurried evaluation and a real plan, not a sales pitch.

You're welcome to share any recent labs you have - they can provide useful context and help Erin understand where you've been. That said, Evangeline Midlife uses a comprehensive hormone and metabolic panel that goes well beyond what most standard providers order. In her experience, outside labs are almost always missing key markers she relies on for a full picture. For that reason, Erin recommends her own panel to make sure that she will have everything she needs to build you the most accurate, personalized protocol possible.

Simply request an appointment for your Comprehensive Hormone Intake package through the online scheduling system. No referral needed. You'll complete intake paperwork before your first visit, and Erin will take it from there.

We understand that life doesn't always go as planned. Because your appointment time is reserved exclusively for you, we ask that you notify us as early as possible if you need to cancel or reschedule.
